Every quarter, C&S BIO IDP students can submit department petitions via MyUCLA Forms to the C&S BIO Undergraduate Office for review. This webpage will be used to facilitate students’ access to the C&S BIO IDP’s MyUCLA Forms. Please find the link that applies to your petition below.
Please note:
- If you click on a MyUCLA forms link and are met with a “The form you are trying to access is not yet open for submissions” statement, then you will need to wait until the window opens.
- New links to MyUCLA Forms will be issued for each submission window. Once the updated links are available, they will be announced on the C&S BIO BruinLearn and shared on this website.
- The Link should be valid as soon as a window opens.

| Contract Course Enrollment Form Link | This form is to submit your C&S BIO research course contract contract(e.g. C&S BIO 195, 198A, 198B, or 199) for course enrollment. A student must first create their contract via MyUCLA and have their PI sign off on the contract. Once the PI has signed the contract, a student must submit the signed contract and syllabus as one PDF file via MyUCLA forms. For additional information, please review this page. | Summer Session; Monday, June 8th – Summer; Friday, June 19th at 11:59 PM. Research course contracts submitted after June 19th are allowed, but are at risk of being added to a student’s study list after the add deadline. | Pre-Major, Major, and Minor Declaration Request Form | The C&S BIO Undergraduate Office will have two program declaration windows for students. Program requests will be collected and reviewed twice per quarter, as well as once during the Summer Session. | Contract Course Tangible Evidence Submission Form | This is to submit your tangible evidence for a contract course you were enrolled in(typically a C&S BIO 195-C&S BIO 199 contract).
